Well, they could have it set up as a way to save the player when things go bad. You wouldn't need to see them appear or anything. Player "dies" and the screen goes black, and then you hear the response team and wake up at the hospital.

Most likely not judging by the little information we have about them.
My guess is that outside the missions that are focused on them or have them involve in any way (Like the gameplay we saw), you'll be able to find a way to get them to a limited number of areas or missions by maybe hacking and giving away a fake signal or finding a random person that may or may not have the insurance and use them as a away of getting an extra side in a firefight.
Not necessarily helping you out, they could be hostile towards everyone except their target with the insurance. Your choices would be either hiding and hoping that they're not gonna see you or you try to take on both your original targets AND the trauma team. Latter would be hard to balance especially if you can loot every gear an entity has on them.
All these ideas would require them to be available at a very specific stage on a map or mission to not be over-used thus making the game too easy.
It's highly improbable though that we'll have anything like this. They are most likely going to only show up in scripted mission specific events.

Possibly. The majority of open world games do exactly that. You die. That's it. Im betting it will just be save/load only because you cant exactly get rescued by trauma team and brought out of an important story mission midway through.
But they would be interesting as a support perk, or maybe you can call in mercs.
